The Masonic Financial Assistance Corporation Financial Assistance Program (MFAC FAP) is not insurance nor does it carry any obligations for payment. MFAC assist it’s participants in coordinating payments through their insurance plans and offers assistance in resolving any remaining balances after their insurance plans have paid. For any balance to be considered for assistance all insurance plans must process the bill first and satisfy their obligations. MFAC FAP will not consider discounts, professional courtesies, or other ineligible charges for assistance.
 
If the participant has any outstanding liability after his insurance company pays please submit an itemized bill containing the following data, all of which is required to consider any assistance.

1. Provider Name, address, and TIN
2. Date of Service
3. Procedure codes or services rendered
4. billed charges
5. copy of insurance explanation of benefits from all carriers

We will normally let you know if any assistance will be offered within 30 days of receiving the material.
